Standing Subcommittee for Human and Social Research Ethics Forms
- Forms Page of the Standing Subcommittee for Human and Social Research Ethics including:
- Research proposal for humanities colleges.
- IRB fee payment undertaking form.
Note: The following courses are required. Certificates for both courses must be attached when submitting requests for approval of research instruments to the Standing Subcommittee for Human and Social Research Ethics.
